What Info To Get When Canvassing

Hi all, I'm looking to create a form to fill out when canvassing. Apart from name and address what info should I be looking to get from the customer and info about the job etc.

i just use my blackberry, I create a new email with the subject called "new customer" and in the email I put down all the details, I dont email it I just save it. (this is great because it shows green in the blackberry"

then when I get home I add all the customers to George and then send the email to myself.

I now have a digital copy on my phone if i ever need to go back for a reference and its also in the George database.

Simple.
