I watched one of your videos on YouTube about a customer info form to take down their details, so made one up myself - do you think I’ve missed anything out..
Didn’t put address, number, website etc.. as it’s just for them to fill out and hand back when I sign them up, but will be making one of these forms you just put up to hand them to keep once I’ve signed them up.

looks good! I would add a section called "notes" this is for anything the customer wants to you do or not do. or for reminding you to text/ call them the night before.
